Hepatopulmonary Syndrome Due to Congenital Extrahepatic Portosystemic Shunts: A Case Report

نویسندگان

چکیده

Congenital extra hepatic port systemic shunt, is a rare clinical entity which can lead to hepatopulmonary syndrome wherein blood from portal vein directly drains into circulation, causing an alteration in metabolism of pulmonary vasoactive substances, vasodilatation, diffusion-perfusion defects, and eventually, arterial hypoxemia due either increase vascular resistance or the creation arteriovenous malformations.

A Rare Cause of Cyanosis: Hepatopulmonary Syndrome Caused by Congenital Extrahepatic Portosystemic Shunt

A 19-year-old male patient presented cyanosis and dyspnoea because of the presence of multiple pulmonary arteriovenous fistulas resulting in oxygen desaturation. The CTA revealed that intestinal and splenic venous blood bypasses the liver and drains into the inferior vena cava. This is the first reported case of hepatopulmonary syndrome caused by congenital extrahepatic portosystemic shunt in w...

Imaging of Congenital Portosystemic Shunts

T he normal portal vascular system transports venous blood from the stomach, intestines, spleen, and pancreas to the hepatic sinusoids and ultimately the hepatic veins and caudal vena cava. Portosystemic shunting (PSS) occurs when abnormal vascular connections occur between the portal and systemic venous systems, bypassing the liver. This can be acquired secondary to portal hypertension arising...

Congenital Extrahepatic Portosystemic Shunt in a Child With Caroli’s Syndrome

A 7-year-old boy presented with recurrent cholangitis, failure to thrive, and progressive splenomegaly. On examination, he had severe pallor, clubbing, and splenohepatomegaly. Lab results showed anemia (hemoglobin 7.2 g/dL) and normal liver functions. Abdominal ultrasound showed hepatosplenomegaly with coarsened liver echotexture and multifocal intrahepatic biliary radical (IHBR) dilatation. Th...

Ultrasonographic evaluation of partially attenuated congenital extrahepatic portosystemic shunts in 14 dogs.

Doppler ultrasonography was used to evaluate the portal vein in 14 dogs before, immediately after and four weeks after a partial ligation of a congenital extrahepatic portocaval shunt. By four weeks after the operation, the hepatofugal or zero flow in the portal vein segment cranial to the shunt origin had become a hepatopetal flow in 13 of the dogs, which became clinically healthy. The other d...

Congenital Extrahepatic Portosystemic Shunts: Spectrum of Findings on Ultrasound, Computed Tomography, and Magnetic Resonance Imaging

Congenital extrahepatic portosystemic shunt (CEPS) is a rare disorder characterised by partial or complete diversion of portomesenteric blood into systemic veins via congenital shunts. Type I is characterised by complete lack of intrahepatic portal venous blood flow due to an end to side fistula between main portal vein and the inferior vena cava. Type II on the other hand is characterised by p...
